A NIOSH study published in August 2026 found that one in three underground coal miners with 25 or more years of experience have black lung disease. The hazard was known. The disease pathway was known. The controls were known. And yet here we are.

As James puts it: if something changes in the process, you are supposed to reevaluate the hazards and the controls to confirm they are still sufficient.
